In this role, you'll support Population Health Management (PHM) programs through the coordination, implementation, and promotion of wellness, preventive care, chronic condition management, maternal health, and member engagement initiatives. You'll serve as a key operational partner across Marketing, Sales, Medical Management, Member Services, and external vendors to ensure our members receive timely, effective, and engaging outreach that inspires healthier outcomes.

What You'll Do
As our Population Health Program Specialist, you will:
- Coordinate and execute population health and wellness campaigns focused on preventive care, maternal health, chronic condition management, and member engagement.
- Manage campaign calendars, communication schedules, and project timelines to ensure successful program delivery.
- Generate, validate, and maintain member outreach lists for email, SMS, newsletters, postcards, and other communication channels.
- Partner with Marketing and internal teams to develop, review, distribute, and track member communications.
- Serve as the primary operational contact for wellness vendors and member engagement partners, coordinating program materials, updates, and communications.
- Coordinate the administration and distribution of member incentive programs and maintain related documentation.
- Support Sales, employer groups, brokers, and internal stakeholders by maintaining educational materials and resources related to Population Health Management (PHM) programs, wellness offerings, and member engagement initiatives.
- Coordinate with Member Services, Marketing, Medical Management, and other internal departments to ensure consistent program information and member messaging.
- Support implementation and ongoing execution of health education, wellness, and member engagement initiatives designed to increase awareness and utilization of PHM programs.
- Monitor campaign activity, participation, and engagement metrics, identifying opportunities to improve member outreach, program awareness, and participation.
- Identify operational, communication, and engagement gaps and provide recommendations to Population Health leadership to improve program effectiveness and member participation.
- Maintain accurate program materials, educational resources, vendor resources, and member-facing content.
- Review communications and educational materials for accuracy and alignment with approved program information.
- Coordinate with print vendors and internal stakeholders to ensure timely production and distribution of outreach materials.
- Collaborate across departments to ensure a seamless and consistent member experience.
- Support continuous improvement efforts by recommending process enhancements and innovative engagement strategies.
What You'll Bring
Required Qualifications
- Bachelor's degree in Public Health, Health Education, Healthcare Administration, Communications, Nutrition, or a related field preferred.
- Minimum of 2–4 years of experience in healthcare, population health, wellness programs, health promotion, member engagement, healthcare communications, or a related field.
- Strong project coordination and organizational skills with the ability to manage multiple priorities, deadlines, and deliverables.
- Excellent written and verbal communication skills.
- Experience working collaboratively with cross-functional teams and external partners.
- Working knowledge of healthcare terminology and population health concepts.
- Experience managing outreach lists, campaign targeting, communication distribution activities, and related reporting.
- Ability to analyze campaign and engagement metrics to identify opportunities for improvement.
- Proficiency with Microsoft Office applications, including Excel, Word, Outlook, PowerPoint, and Teams.
Preferred Qualifications
- Certified Health Education Specialist (CHES), Wellness Coach, Medical Assistant (CMA/RMA), or related certification.
- Experience with healthcare systems, reporting platforms, business intelligence tools, Cognos, Facets, dashboards, or similar technologies.
- Experience supporting wellness, preventive care, health education, or population health initiatives.
